Whether you’re at risk for diabetes, you’ve recently been diagnosed or you’ve been living with diabetes for years, Teladoc Health can help you improve your health. The personalized, data-driven digital program can help you understand your blood sugar, develop healthy lifestyle habits and maintain glycemic control.
Highlights
- You and your dependents can use the program at no cost if you’re enrolled in a UnitedHealthcare (UHC) medical plan and aged 18 or older. (If you’re a Kaiser member, you aren’t eligible, but you have access to many resources, classes and tools through Kaiser.)
- Once you’re registered with Teladoc Health, a connected blood glucose meter—along with unlimited strips and lancets—will be shipped to your home. The meter can connect directly to the Teladoc app.
- Through the app, you can monitor your readings and export them to your health care provider, track your goals, and access one-on-one live or virtual coaching.
- If you log an abnormal glucose reading, you can connect virtually with a medical expert.

Start using Teladoc Health by registering through the Teladoc website, downloading the Teladoc app or calling 1-800-TELADOC. Use company code SPLUNK. You’ll be asked a series of intake questions, including your UHC group ID (902860) and member ID.
