What you need to know

If you’re ever affected by a critical illness or a surprise hospital stay, you (and your bank account) will be glad you put some safety nets in place. Critical illness and hospital indemnity are optional plans that pay cash benefits directly to you if you experience a covered event, regardless of what your medical plan pays. Premiums are based on your age, and you pay the full cost. You can enroll yourself as well as your spouse and/or children.

Critical illness insurance

This coverage provides:

  • A cash benefit if you’re diagnosed with a covered critical illness or disease, whether or not the charges are covered by your medical plan
  • Benefit payments sent directly to you that can be used for any purpose, from covering medical copays and deductibles to paying for everyday expenses such as the mortgage, groceries and utilities
  • Simple enrollment with no health or medical questions to answer (when you sign up for coverage during new hire enrollment)
  • Limitations on preexisting conditions; contact Guardian if you have questions.

Covered critical illnesses include:

  • Heart attack
  • Multiple sclerosis
  • Stroke
  • Alzheimer’s disease
  • Parkinson’s disease
  • Major organ failures

Your coverage options

  Benefit coverage amounts you can choose
Employee Increments of $5,000, up to $30,000
Spouse Increments of $2,500, up to $15,000 (up to 50% of employee benefit)
Children 25% of employee benefit

Hospital indemnity insurance

This coverage provides:

  • A cash benefit when you’re admitted to a hospital for any reason, whether or not the charges are covered by your medical plan
  • A cash benefit when you get a covered annual health screening
  • Benefit payments sent directly to you that can be used for any purpose, from covering medical copays and deductibles to paying for everyday expenses such as the mortgage, groceries and utilities
  • Simple enrollment with no health or medical questions to answer
  • The ability to take the coverage with you if you change jobs or retire

What the plan pays

Coverage Benefit
Hospital admission $1,500 per admission
Maximum: 1 admission per year, per insured (3 admissions per year, per covered family)
Hospital/ICU confinement $100 per day
Maximum: 15 days per year

Note: There is a preexisting condition limitation that excludes coverage for conditions for a certain time prior to and after you first enroll. Check with Guardian for details.
